Abstract

Kontraktur sendi pasca luka bakar terjadi akibat pemendekan dan pengerasan jaringan parut selama proses penyembuhan, yang sering melibatkan fase inflamasi, proliferasi, kontraksi, dan remodeling. Di negara berkembang seperti Indonesia, keterbatasan akses rehabilitasi dan tindak lanjut yang kurang optimal meningkatkan risiko kontraktur permanen. Tulisan ini bertujuan mengkaji definisi, patofisiologi, klasifikasi, pencegahan, dan terapi kontraktur sendi pada pasien riwayat luka bakar untuk memperkuat dasar ilmiah dalam peningkatan pelayanan dan pencegahan kecacatan jangka panjang. Metode yang digunakan adalah tinjauan pustaka dari sumber primer dan sekunder terkini, termasuk pedoman WHO, jurnal bedah plastik rekonstruksi, serta studi lokal dan internasional tentang evaluasi dan manajemen kontraktur. Kontraktur dapat diklasifikasi menjadi ekstrinsik dan intrinsik, serta menurut jaringan penyebab (dermogen, tendogen, arthrogen). Penatalaksanaan meliputi pencegahan primer hingga tersier: pembidaian, posisi preventif, latihan rentang gerak, terapi tekanan, pijat, dan pelembap. Bila kontraktur sudah terbentuk, intervensi operatif seperti Z-plasty, skin graft, flap lokal atau free flap, serta ekspansi jaringan menjadi pilihan, didukung rehabilitasi multidisiplin. Pendekatan multidisiplin yang dimulai sejak fase akut luka bakar krusial untuk mencegah kontraktur. Intervensi pencegahan dan kuratif yang tepat waktu dapat meminimalkan disabilitas jangka panjang dan meningkatkan kualitas hidup pasien.
